Job Summary
Deliver skilled, patient-centered care to patients in their homes in the Marysville, PA area, performing comprehensive assessments including vital signs, medication management, and wound care. Educate patients and caregivers on disease management and prescribed treatments while maintaining accurate EMR documentation. Travel to assigned patient locations within your territory, working independently with an interdisciplinary team support. Requires an active LPN license, two years of experience, and a current driver's license to spend approximately 20% of your day driving. This role offers a $10,000 first-year success bonus and eligibility for a pathway to an ASN degree with up to $20,000 in student loan repayment.
Required Qualifications
- Active Licensed Practical Nurse (LPN) license in the state you work
- Two years of LPN experience
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Must be legally authorized to work in the United States
