Provide skilled nursing care in the home environment for young, middle, and older adults, evaluating patient status at each visit and communicating findings to MD/RN; educate patients, families, and caregivers on nursing care, medications, diet, and self-care; advocate for patients and coordinate with health care providers to ensure comprehensive care; maintain licensure and participate in ongoing staff development; document within 48 hours, manage time and productivity, and adhere to the agency's CARE/ethical standards; assist with orientation of new LVN staff and collaborate with RN to coordinate care; engage in quality improvement activities and support the organization’s values and culture.
