Job Summary
Deliver quality patient care in compliance with physician orders under direction of their Registered Nurse teammate. Perform the continuum of hands-on client care through documentation and timely communication with the client's physician and other caregivers. Recognize changes in patient needs and responses requiring intervention to prevent or reduce risk. Educate clients and their family members based on specific needs while providing care in patients' homes using skills such as phone triage, wound care, infusions, and catheter care. Schedule includes part-time PRN visits Monday through Friday in Columbia and Suwannee.
Required Qualifications
- An active LPN License in the state of application
- Valid CPR
- Valid drivers license
- Must be able to lift 50 lbs
- Must be available for weekend shifts
- Must be able to effectively lead and coach others

Desired Qualifications
- Medicare Skilled Nursing experience
- Basic understanding of Oasis
- 1-year LPN experience in a home health or health care setting
- Home Care Home Base exp a plus
