Job Summary
Deliver quality patient care in compliance with physician orders, performing hands-on client care including wound care, infusions, catheter care, and PICC line dressing changes in patients' homes. Recognize changes in patient needs requiring intervention and implement care to prevent or reduce risk while educating clients and family members based on specific needs. Document skilled nursing care accurately and communicate timely updates with the client's physician and other caregivers. Maintain confidentiality and professional boundaries throughout all interactions. Requires an active LPN license, valid CPR, and drivers license, with preferred experience in Medicare Skilled Nursing and home health settings.
Required Qualifications
- An active LPN License in the state of application
- Valid CPR
- Valid Drivers License
- Must be able to deliver quality patient care in compliance with physician orders
- Must be able to perform the continuum of hands on client care through documentation and timely communication
- Must be able to recognize changes in patient needs and responses requiring intervention
- Must be able to educate clients and their family members based on client's specific needs
- Must be able to provide care in patients home using a variety of skills such as phone triage, patient education, observation/assessment, wound care, infusions, catheter care, PICC line dressing changes
- Must have 1-year LPN experience in a home health or health care setting
Desired Qualifications
