Job Summary
Provide direct patient care as a Home Health LPN in a home-health setting, following the plan of care developed by the Registered Nurse and coordinating with the care team. Transcribe physician orders as directed, forward clinical and progress notes to the RN, attend patient care conferences, and documentation must be thorough and timely. Requires current LPN licensure in the state of the branch and at least one year of clinical nursing experience (preferably in an acute care setting); CPR certification required. Also requires completion of nursing education from an approved school of nursing and effective communication skills.
Required Qualifications
- Current LPN licensure in the state in which the branch is located
- one year of clinical nursing experience, preferably in an acute care medical/surgical setting
- CPR certification
- Successful completion of nursing education from an approved school of nursing
