Job Summary
Provide home-based care to patients with illnesses, injuries, and chronic diseases, often after hospital discharge, under RN supervision. Take patient vitals, perform assessments, document progress, and report changes to the RN supervisor. Assist RNs in carrying out the patient's plan of care, teaching self-care techniques, and educating families on medication and home safety. Collaborate with physicians, RNs, CNAs, and aides to coordinate patient care and plan for discharge. Maintain an active, unrestricted Michigan license with at least one year of clinical experience and current CPR certification. Work in a family-owned environment prioritizing work-life balance and autonomy.
Required Qualifications
- Licensed Practical Nurse (LPN) with active, unrestricted license in the State of Michigan
- At least one (1) year of clinical experience as a Licensed Practical Nurse (LPN)
- Current CPR Certification
- Reliable transportation with a valid state driver's license and current car insurance
