Job Summary
LPN Intake Clinical Coordinator in home health to oversee clinical intake, triage, and coordination of services. Responsible for reviewing clinical documentation, supporting families and providers, assisting in policy development, participating in on-call rotations, and contributing to accreditation and payer compliance. Role is in-office Monday–Friday with travel as needed; requires active LPN license, CPR, valid driver’s license, and EMR experience (HCHB preferred).
Required Qualifications

- Available for on-call scheduling
