Job Summary
Provide professional nursing care in clients’ homes under the direction of a Registered Nurse, performing skilled nursing visits as directed by the RN Case Manager, and following the established plan of care. Maintain accurate documentation in the clinical record, communicate status changes promptly, promote safety, ensure confidentiality, participate in Quality Improvement activities, coordinate home care services, and support a team-oriented work environment with minimal supervision. Must be a graduate of an accredited nursing program with an Indiana LPN license, CPR certification, at least 1 year of acute care experience, strong communication skills, a valid driver's license, and the ability to complete electronic documentation.
Required Qualifications
- Graduate of an accredited school of nursing
- Current Licensed Practical Nurse (LPN) license in the State of Indiana
- Minimum of one (1) year of experience in an acute care setting or equivalent experience
- Current CPR certification
- Strong written, verbal, and interpersonal communication skills
- Valid driver’s license with a reliable, insured vehicle in accordance with state and agency requirements
- Ability to complete electronic/paperless documentation
