Job Summary
Deliver skilled, patient-centered care to patients in their homes in the Green Tree area, performing comprehensive assessments including vital signs, medication management, and wound care. Educate patients and caregivers on disease management and prescribed treatments while maintaining accurate, timely EMR documentation. Work independently within an assigned territory, traveling to patient locations to provide skilled nursing support in collaboration with an interdisciplinary team. This role offers a $10,000 first-year success bonus and includes a pathway to an ASN degree with up to $20,000 in student loan repayment.
Required Qualifications
- Active Licensed Practical Nurse (LPN) license in the state you work
- Two years of LPN experience
- Clinical expertise to deliver quality patient care
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Successful completion of a drug screen
- Confirmation that you are legally authorized to work in the United States
