Job Summary
Home Health LPN provides management and delivery of patient care plans in patients’ homes as part of a team-based AHN Healthcare@Home. The role supports Cranberry Township and surrounding areas, with an emphasis on timely charting and EMR documentation, traveling to patient locations (~20% of the day). Must have an LPN license for the state of work, graduate from an approved practical nursing program, at least two years of LPN experience, and a valid driver’s license. The position requires comfort with tablets and smartphones for care coordination and use of EMR software, and adherence to deadlines within a home health care setting. Required Qualifications
- LPN license in the state you work (Pennsylvania)
- Graduate from an approved school of practical nursing
- Two years of LPN experience
