Home Health LPN

Licensed Practical Nurse (LPN) for a Home Health position serving patients in their homes in the Mechanicsburg, PA area. Deliver skilled, patient-centered home health care in collaboration with an interdisciplinary team. Conduct comprehensive patient assessments (vital signs, medications, wound care, overall health status), educate and support patients and caregivers on disease management and prescribed treatments to promote adherence, and maintain accurate EMR documentation. Work independently in the patient homes with ~20% daily driving to/from patient locations within an assigned territory.
