Job Summary
Provide management and delivery of patient care plans in home health as part of AHN Healthcare@Home, collaborating in a team-based environment. The role requires an LPN license in the working state, a minimum of two years of LPN experience, and the ability to drive ~20% of the day to patient locations. The nurse will self-manage time and scheduling, use tablets and EMR software for charting, and participate in an onboarding-oriented, supportive environment with clinical educators and supervisors. Required Qualifications
- LPN license in the state you work
- Graduate from an approved school of practical nursing
- Two years of LPN Experience
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations

- The skills needed to self-manage your time and schedule
- Demonstrated experience with tablets, mobile phones and EMR software
