Job Summary
LPN Clinical Intake Coordinator leading home health operations with in-office schedule Monday–Friday. Responsibilities include assessing and triaging client concerns, coordinating services with field staff and leadership, reviewing clinical documentation, communicating with families and physicians to support client outcomes, assisting in policy development to ensure regulatory adherence, filling staffing gaps, participating in on-call rotations, and driving process improvements. Manage medical supplies cost-effectively, prepare case reviews, and help ensure accreditation and payer compliance.
Required Qualifications
- Active LPN license (state-specific)
- CPR certified
- Valid driver’s license, insurance, reliable transport, clean driving record
- Available for on-call scheduling
- Willing to travel as needed
- Experience in home care (preferred)
- Knowledge of Medicare and accreditation standards (preferred)
- EMR savvy (HCHB preferred)
- Geriatric Home Health experience preferred
