Home Health LPN Clinical Referral Coordinator Full Time

LPN Clinical Intake Coordinator, a full-time in-office role (Mon-Fri) focused on clinical intake, oversight, and coordination for home healthcare. Responsibilities include assess/triage and resolve client concerns; collaborate with field staff and leadership to coordinate services and review clinical documentation; communicate with families, physicians, and providers to support client outcomes; assist in policy development and ensure adherence to regulations and protocols; fill in for staffing gaps and participate in on-call rotations; propose process improvements and manage medical supplies cost-effectively; prepare case reviews and assist with accreditation and payer compliance. Requires active LPN license, CPR, valid driver’s license, willingness to travel, and EMR experience (HCHB preferred); travel as needed and geriatric home health experience preferred. In-office location: Granite City, IL.
