Job Summary
Coordinate patient care services and clinical operations by reviewing HCHB notes, processing administrative workflows, and entering physician orders into the appropriate system. Maintain the At-Risk Registry, perform chart audits, verify Medicare notices of non-coverage, and assist with scheduling follow-up appointments and pre-authorizations. Collaborate with clinicians, physicians, and office staff to ensure smooth care transitions, while performing field visits or direct patient care activities as needed. Work full-time, Monday through Friday, in an in-office position in Braintree, MA.
Required Qualifications
- Graduate of an accredited school of nursing
- Current, unrestricted LPN license in the state of Massachusetts
- Minimum of one year of nursing experience in a clinical setting
- Strong organizational skills and attention to detail
- Excellent communication and teamwork abilities
- Comfortable working in a fast-paced office environment while managing multiple priorities
- Ability and willingness to travel within the branch coverage area if needed
- Valid driver's license, reliable transportation, and current auto insurance in accordance with state requirements
- Ability to occasionally lift, push, pull, and support up to 50 pounds, including assisting with equipment or patient-related activities when necessary
