Home Health Licensed Vocational Nurse

Provide skilled nursing care to patients onsite in their homes under the direction of an RN, implementing individualized care plans in collaboration with the patient, family, and healthcare team. Educate patients and families on disease management and self-care techniques while maintaining accurate documentation of visits, interventions, and outcomes. Maintain prompt contact with physicians and clinical managers regarding patient needs and status changes, participating in care coordination and discharge planning activities. Work onsite in patient homes as part of a dedicated team focused on helping patients regain independence.
