Job Summary
Provide skilled nursing care to patients in the home setting under the supervision of a Registered Nurse. Assist in identifying patient physical, psychosocial, and environmental needs while administering treatments and interventions per physician orders. Participate in developing the Plan of Care, monitor patient conditions, and report changes to the supervising RN. Maintain accurate clinical documentation, follow infection control protocols, and attend interdisciplinary case conferences. Comply with state regulations and complete required in-service training. Meet productivity expectations and ensure timely documentation.
Required Qualifications
- Licensed Vocational Nurse (LVN) in the state of Texas
- Strong knowledge of patient care, infection control, and documentation practices
- Ability to follow and implement a Plan of Care (POC) under RN supervision
- Strong clinical assessment, documentation, and communication skills
- Ability to identify and report changes in patient condition
- Reliable transportation with valid driver's license and current auto liability insurance
- Ability to read, write, and communicate effectively in English
Desired Qualifications
- Minimum of 1 year of home health experience preferred
- Experience working with interdisciplinary teams in a home health setting preferred
