Job Summary
Provide skilled nursing care to patients onsite in their homes under the direction of an RN, implementing individualized care plans and educating families on disease management and self-care. Maintain accurate documentation of visits, interventions, and outcomes while coordinating with physicians and the clinical team to manage care plans and discharge planning. Participate in care coordination activities and promptly report patient or family status changes. Requires current nursing license, valid driver's license, CPR certification, and two years of LPN/LVN experience. Pay ranges from $53,000 to $73,000 annually based on per visit rates.
Required Qualifications
- Current nursing license in the practicing state
- Valid drivers license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
- Valid state driver's license
- Personal vehicle liability insurance
- Vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher
- Required to be screened for TB
