Job Summary
Provide skilled nursing care to patients in their homes under the direction of an RN. Implement individualized care plans in collaboration with the patient, family, and healthcare team. Educate patients and their families on disease management, treatment options, and self-care techniques. Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes. Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes. Participate in care coordination activities and discharge planning. Requires current nursing license, valid driver’s license, CPR certification, and two years of LPN/LVN clinical experience. Pay is presented as per-visit and base annual pay; role is part-time and home-health nursing in League City, TX.
Required Qualifications
- Current nursing license in the practicing state
- Valid driver’s license, auto insurance and reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
