Home Health Licensed Practical Nurse - PPV

Home Health Licensed Practical Nurse (LPN) provides at-home patient care to support recovery and enable patients to remain at home. Responsibilities include assisting with care, educating patients and families on disease management, and empowering patients to reach health goals. Required qualifications include graduation from an accredited Practical Nursing program, current LPN license, valid driver’s license, CPR certification, and a negative TB screen with hepatitis consent/declination. Two years of LPN clinical experience (home health preferred) are required. The role is based in Kokomo, IN, and requires in-person patient visits as part of a home health team.
