Home Health Licensed Practical Nurse-Per Diem

Provide skilled nursing care to patients onsite in their homes under the direction of an RN. Implement individualized care plans in collaboration with the patient, family, and healthcare team. Educate patients and families on disease management, treatment options, and self-care techniques. Maintain accurate records of patient visits, interventions, and outcomes. Maintain contact with physicians, clinical managers, and other team members regarding patient needs and status changes. Participate in care coordination activities and discharge planning. This role supports patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization.
