Home Health Licensed Practical Nurse LPN PRN Part Time

Job Summary
Deliver quality patient care in compliance with physician orders under the direction of a Registered Nurse teammate. Perform the continuum of hands-on client care through documentation and timely communication with the client's physician and other caregivers. Recognize changes in patient needs requiring intervention and implement care to prevent or reduce risk. Educate clients and their family members based on specific needs while providing wound care, infusions, catheter care, and phone triage in patients' homes. This part-time Monday through Friday schedule includes an active LPN license, valid CPR, and one year of home health experience.
Required Qualifications
- An active LPN License in the state of application
- Valid CPR
- Must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
Desired Qualifications
- Medicare Skilled Nursing experience
- Basic understanding of Oasis
- 1-year LPN experience in a home health or health care setting
- Home Care Home Base exp a plus
