Job Summary
Deliver quality patient care in compliance with physician orders, performing hands-on client care including wound care, infusions, catheter care, and PICC line dressing changes. Recognize changes in patient needs requiring intervention and educate clients and family members based on specific needs. Provide care in patients' homes using phone triage and observation/assessment skills while documenting care plans and communicating timely with physicians and caregivers. Active LPN license and valid CPR required; Medicare Skilled Nursing experience preferred. North Miami Dade County territory.
Required Qualifications
- An active LPN License in the state of application
- Valid CPR
- Must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons as appropriate
Desired Qualifications
- Medicare Skilled Nursing experience
- Basic understanding of Oasis
- 1-year LPN experience in a home health or health care setting
- Home Care Home Base exp a plus
