Job Summary
Provide skilled nursing care under the direct supervision of the Clinical Supervisor; perform evaluations including vital signs and medication reviews; coordinate with physicians, therapists, and others in the client's care plan; assess and chart observations of the client’s condition at each visit; maintain complete and accurate nursing documentation; educate clients and families on proper home health care procedures; monitor and report changes in patients’ conditions; collaborate with the health care team to assess, plan, implement, or evaluate patient care plans; modify treatment plans as indicated by patient responses; assess home or work environments to identify potential health or safety problems; perform other duties as assigned.
Required Qualifications
- Associates degree required
- Graduate of an accredited state approved, Bachelor’s Degree from school of nursing preferred
- Current and valid Virginia Licensed Practical Nurse license
- One year of LPN experience
- Current CPR certification
- Driver’s License
- Car Insurance
- Car Registration
- Home health care experience
- Bilingual (Spanish) preferred
