Job Summary
Provide skilled nursing care in patients’ homes under the direction of an RN/physician; Monitor and document patient condition and response to treatment; Administer medications and treatments as ordered; Educate patients and families on care plans and disease management; Assist with coordination of care among interdisciplinary team members; Complete timely and accurate clinical documentation.
Required Qualifications
- Active Oklahoma LPN License
- Current BLS/CPR Certification
- Valid Driver’s License and Reliable Transportation
- Home Health Experience Preferred
