​Home Health Licensed Practical Nurse (LPN) – Full-Time

Provide skilled nursing care in patients' homes under the direction of the Registered Nurse and physician. Perform nursing procedures and treatments in accordance with the patient's plan of care, monitor conditions, and promptly communicate changes to supervising staff. Administer medications and treatments as prescribed while educating patients and caregivers regarding medications, disease processes, safety, and home care techniques. Accurately complete clinical documentation in a timely manner and collaborate with physicians, nurses, therapists, and other healthcare team members to maintain compliance with all state, federal, Medicare, and agency regulations.
