Job Summary
Provide and document skilled nursing care in patients' homes under physician orders and supervision, following developed care plans; perform continuum of hands-on client care, document care, and communicate with the patient's physician and other caregivers; recognize changes in patient needs and implement care to prevent or reduce risk; educate clients and family members; deliver care using skills such as phone triage, wound care, infusions, catheter care, PICC line dressing changes, and other home-health tasks; adhere to confidentiality and professional boundaries.
Required Qualifications
- An active LPN License in the state of application
- Valid CPR
- Valid Drivers License
