Job Summary
Provide skilled nursing care in the home-health setting under the supervision of a Registered Nurse; administer medications and treatments as ordered; perform patient assessments and assist with care planning; educate patients and caregivers on disease management and care plans; monitor patient condition and report changes to the supervising RN; collaborate with physicians and interdisciplinary teams; complete documentation using streamlined, AI-assisted tools (WellSky/Kinnser); flexible scheduling options including full-time/PRN/modified schedules; 1:1 patient care with a focus on autonomy and meaningful care in the home; coverage area includes Westchase/West Hillsborough County, FL; leveraging technology to streamline charting and documentation; benefits include health insurance, PTO, 401(k), mileage reimbursement, data plan reimbursement, EAP, and referral bonuses.
Required Qualifications
- Active, unencumbered Florida Licensed Practical Nurse (LPN) license
- Valid driver’s license, reliable transportation, and auto insurance
- CPR certification
- Home health experience preferred, but not required
