Job Summary
Provide skilled nursing visits in patients' homes under RN supervision, delivering one-on-one care and promoting independence. Responsibilities include wound care, medication management, disease education, patient and caregiver education on self-care, collaboration with RN Case Managers and physicians, accurate EMR documentation, and implementing care plans to improve outcomes and reduce avoidable hospitalizations. Must be a Graduate of an accredited LPN program with an active LPN license, have at least 1 year of clinical nursing experience, and be able to travel within the service area; driving and transportation are required. Full-time schedule with visits to patients in the home setting and coordination with the care team to ensure quality home health care across Massachusetts (Springfield and Northampton areas).
Required Qualifications
- Graduate of an accredited Practical Nursing program
- Current, unrestricted Licensed Practical Nurse (LPN) license
- Minimum of one (1) year of nursing experience in a clinical setting
- Valid driver's license, reliable transportation, and current auto insurance
- Ability to travel throughout the assigned service area
- Strong communication, assessment, and organizational skills
