Job Summary
Deliver quality patient care in compliance with physician orders, performing hands-on client care including wound care, infusions, catheter care, and phone triage. Document skilled nursing care while recognizing changes in patient needs to prevent or reduce risk, and educate clients and family members based on specific needs. Provide care in patients' homes across Shelby, Jefferson, and Chilton counties under the direction of a Registered Nurse teammate. Requires an active LPN license, valid CPR, and drivers license.
