Job Summary
Provide skilled nursing care to patients onsite in their homes under RN direction, implementing individualized care plans and educating families on disease management and self-care. Maintain accurate documentation of visits and interventions while coordinating with physicians and the clinical team for care planning and discharge activities. Requires current LPN license, two years clinical experience, valid driver's license, and CPR certification. Pay ranges from $53,000 to $73,000 annually for full-time roles.
Required Qualifications

- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
- Valid state driver's license
- Personal vehicle liability insurance
- Vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher
- TB screening
