Job Summary
Provide skilled nursing care to patients onsite in their homes under RN direction while implementing individualized care plans. Collaborate with patients, families, and the healthcare team to manage recovery and educate on disease management and self-care. Maintain accurate documentation of visits and interventions, then coordinate with physicians and clinical managers regarding status changes and discharge planning. Requires a current nursing license, two years of LPN/LVN experience, and a valid driver's license. Full-time 40-hour schedule with an annual base pay range of $53,000 to $73,000.
Required Qualifications
- Current nursing license in the practicing state
- Valid drivers license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
- TB screening
- Valid state driver's license
- Personal vehicle liability insurance
- Vehicle insurance in accordance with residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher
