Job Summary
Provide skilled nursing care to patients onsite in their homes under RN direction, implementing individualized care plans in collaboration with the patient, family, and healthcare team. Educate patients and families on disease management, treatment options, and self-care techniques while maintaining accurate documentation of visits, interventions, and outcomes. Maintain prompt contact with physicians and clinical managers regarding patient status changes, participating in care coordination and discharge planning activities. Requires a current nursing license, two years of LPN/LVN clinical experience, valid driver's license, and CPR certification. Pay ranges from $53,000 to $73,000 annually for full-time 40-hour weeks.
