Home Health Licensed Practical Nurse

Provide skilled nursing care to patients onsite in their homes under RN direction. Implement individualized care plans in collaboration with the patient, family, and healthcare team. Educate patients and families on disease management, treatment options, and self-care techniques. Maintain accurate records of patient visits, interventions, and outcomes. Maintain contact with physicians and clinical managers regarding patient needs and status changes. Participate in care coordination activities and discharge planning. This role requires a current nursing license, two years of LPN/LVN clinical experience, and a valid driver's license with auto insurance.
