Job Summary
Provide skilled nursing care to patients onsite in their homes under RN direction, implementing individualized care plans and educating families on disease management and self-care. Maintain accurate documentation of visits and interventions while coordinating with physicians and the clinical team to address status changes and discharge planning. Requires a current nursing license, two years of LPN/LVN clinical experience, CPR certification, and valid driver's license with auto insurance. Located in Naples, Marco, Bonita Springs, and Estero areas with a 40-hour weekly schedule and annual base pay ranging from $53,000 to $73,000.
Required Qualifications
- Current nursing license in the practicing state
- Valid drivers license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
- Must be screened for TB
- Must maintain personal vehicle liability insurance
- Must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher
