Job Summary
Provide skilled nursing care to patients onsite in their homes under RN direction, implementing individualized care plans in collaboration with the patient, family, and healthcare team. Educate patients and families on disease management, treatment options, and self-care techniques while maintaining accurate documentation of visits, interventions, and outcomes. Maintain prompt contact with physicians and clinical managers regarding patient needs and status changes, actively participating in care coordination and discharge planning. Requires a current nursing license, valid driver's license, and two years of LPN/LVN clinical experience. Full-time 40-hour weekly schedule with pay per visit ranging from $33.00 to $47.00.
Required Qualifications
- Current nursing license in the practicing state
- Valid drivers license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
- TB screening
- Valid state driver's license
- Personal vehicle liability insurance
- Vehicle insurance in accordance with residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher
