Job Summary
Provide skilled nursing care to patients onsite in their homes under RN direction, implementing individualized care plans in collaboration with the patient and family. Educate patients and families on disease management and self-care techniques while maintaining accurate documentation of visits, interventions, and outcomes. Maintain prompt contact with physicians and the clinical team regarding patient status changes, participating in care coordination and discharge planning. Requires a current nursing license, two years of LPN/LVN experience, and a valid driver's license with auto insurance. Pay ranges from $58,300 to $80,300 annually for full-time employment.
Required Qualifications
- Current nursing license in the practicing state
- Valid drivers license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
- TB screening
- Valid state driver's license
- Personal vehicle liability insurance
- Vehicle insurance in accordance with residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher
