Home Health Licensed Practical Nurse

Provide skilled nursing care to patients onsite in their homes under RN direction while implementing individualized care plans. Collaborate with patients, families, and the healthcare team to manage care plans supporting recovery, and educate families on disease management and self-care techniques. Maintain accurate documentation of visits, interventions, and outcomes, and promptly contact physicians and clinical managers regarding patient status changes. Participate in care coordination activities and discharge planning. Requires a current nursing license, valid driver's license, and two years of LPN/LVN clinical experience.
