Job Summary
Provide skilled nursing care to patients onsite in their homes under the direction of an RN. Implement individualized care plans in collaboration with the patient, family, and healthcare team to support recovery. Educate patients and families on disease management, treatment options, and self-care techniques. Maintain accurate records of patient visits, interventions, and outcomes. Maintain contact with physicians and the clinical manager regarding patient needs and status changes. Participate in care coordination activities and discharge planning. This role offers a pay-per-visit rate of $33.00 to $47.00, with an annualized range of $53,000 to $73,000 for 40 weekly hours. Candidates must hold a current nursing license, valid driver's license, and CPR certification, with two years of LPN/LVN experience required.
Required Qualifications

- Personal vehicle liability insurance
- Vehicle insurance in accordance with residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher
