Home Health Licensed Practical Nurse

Provide skilled nursing care to patients onsite in their homes under the direction of an RN. Implement individualized care plans in collaboration with the patient, family, and healthcare team. Educate patients and their families on disease management, treatment options, and self-care techniques. Maintain accurate records of patient care, including documentation of visit interventions, and outcomes. Maintain contact with physicians, clinical managers, and the healthcare team regarding patient needs and status changes. Participate in care coordination activities and discharge planning. Work 40 weekly hours at a pay rate of $33.00 to $47.00 per visit.
