Job Summary
Provide skilled nursing care to patients onsite in their homes under the direction of an RN. Implement individualized care plans in collaboration with the patient, family, and healthcare team. Educate patients and families on disease management, treatment options, and self-care techniques. Maintain accurate records of patient visits, interventions, and outcomes. Maintain prompt contact with physicians, clinical managers, and other team members regarding patient needs and status changes. Participate in care coordination activities and discharge planning. Requires current nursing license, two years of LPN/LVN clinical experience, and valid driver's license. Full-time 40-hour schedule with base pay ranging from $53,000 to $73,000 annually.
Required Qualifications
- Current nursing license in the practicing state
- Valid drivers license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
- Valid state driver's license
- Personal vehicle liability insurance
- Vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher
- Required to be screened for TB
