Job Summary
Provide skilled nursing care to patients onsite in their homes under the direction of an RN. Implement individualized care plans in collaboration with the patient, family, and healthcare team. Educate patients and families on disease management, treatment options, and self-care techniques. Maintain accurate records of patient visits, interventions, and outcomes. Maintain prompt contact with physicians, clinical managers, and the healthcare team regarding patient needs. Participate in care coordination activities and discharge planning. Work onsite in the comfort of patients' homes to help them regain independence.
Required Qualifications
- Current nursing license in the practicing state
- Valid drivers license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
- Valid state driver's license
- Personal vehicle liability insurance
- Vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher
- Required to be screened for TB
