Home Health Licensed Practical Nurse

Provide skilled nursing care to patients onsite in their homes under the direction of an RN. Implement individualized care plans in collaboration with the patient, family, and healthcare team. Educate patients and families on disease management, treatment options, and self-care techniques. Maintain accurate records of patient visits, interventions, and outcomes. Maintain contact with physicians, clinical managers, and other team members regarding patient needs and status changes. Participate in care coordination activities and discharge planning. This role is patient-facing and requires TB screening. Requires a current nursing license, valid driver's license, CPR certification, and two years of LPN/LVN clinical experience. Pay ranges from $53,000 to $73,000 annually.
