Job Summary
Provide skilled nursing care to patients onsite in their homes under RN direction while implementing individualized care plans. Collaborate with patients, families, and the healthcare team to manage care plans supporting recovery and educate on disease management and self-care. Maintain accurate documentation of visits, interventions, and outcomes, and promptly coordinate with physicians and clinical managers regarding patient status changes. Participate in care coordination activities and discharge planning. Requires current nursing license, two years of LPN/LVN clinical experience, and valid driver's license with auto insurance.
Required Qualifications
- Current nursing license in the practicing state
- Valid drivers license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
- TB screening
- Valid state driver's license
- Personal vehicle liability insurance
- Vehicle insurance in accordance with residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher
