Job Summary
Provide skilled nursing care to patients onsite in their homes under the direction of an RN, implementing individualized care plans and educating families on disease management and self-care. Maintain accurate documentation of visits and interventions while coordinating with physicians and the clinical team regarding patient status changes. Participate in care coordination activities and discharge planning to support patient recovery. This role requires a current nursing license, valid driver's license, and two years of LPN/LVN clinical experience. Pay is per visit/unit ranging from $33.00 to $47.00, with scheduled weekly hours of 40. Required Qualifications
- Current nursing license in the practicing state
- Valid drivers license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
- Coverage needed in Licking County
- Valid state driver's license
- Personal vehicle liability insurance
- Vehicle insurance in accordance with residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher
- Required to be screened for TB
