Job Summary
Provide skilled nursing care to patients in their homes under RN direction, implementing individualized care plans and educating families on disease management and self-care. Maintain accurate documentation of visits, interventions, and outcomes while coordinating with physicians and the clinical team to manage patient needs and discharge planning. Requires a current nursing license, two years of LPN/LVN clinical experience, valid driver's license, and CPR certification. Full-time 40-hour schedule with annual base pay ranging from $53,000 to $73,000.
Required Qualifications

- TB screening
- Valid state driver's license
- Personal vehicle liability insurance
- Vehicle insurance in accordance with residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher
