Job Summary
As a Home Health Licensed Practical Nurse with CenterWell, you will provide skilled nursing care to patients in their homes under RN direction, implement individualized care plans, educate patients and families on disease management and self-care, maintain accurate documentation of patient care, and coordinate with healthcare team members. This role requires a current nursing license, CPR certification, a valid driver's license, auto insurance, and two years of clinical LPN/LVN experience.
