Home Health Licensed Practical Nurse

Provide skilled nursing care to patients in their homes under the direction of an RN; implement individualized care plans in collaboration with patient, family, and healthcare team; educate patients and families on disease management and self-care; maintain accurate patient care documentation; maintain communication with the healthcare team; participate in care coordination and discharge planning; role is patient-facing and TB screening may be required.
