Job Summary
Provide skilled nursing care to homebound patients under the direction of an RN in patients' homes. Implement individualized care plans, educate patients and families on disease management and self-care, maintain accurate patient care documentation, coordinate with physicians and the healthcare team, and participate in discharge planning. Requires active LPN/LVN license in practicing state, valid driver’s license with auto insurance, CPR certification, and at least two years of clinical LPN/LVN experience. TB screening and driver-safety requirements apply.
Required Qualifications
- Current nursing license in the practicing state
- Valid drivers license, auto insurance and reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
