Job Summary
Provide skilled nursing care to patients in their homes under the direction of an RN, implementing individualized care plans and educating patients and families on disease management and self-care. Maintain accurate documentation of patient visits, interventions, and outcomes; coordinate with physicians and the clinical team; participate in discharge planning and ongoing care coordination to support patient recovery and independence.
Required Qualifications
- Current nursing license in the practicing state
- Valid drivers license, auto insurance and reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
